The season 12 contestant was in a fiery ultralight aircraft crash on Monday.

Erin Storm, who vied for Matt Grant's heart during season 12 of The Bachelor in 2008, reportedly died in a plane crash on Monday. The Los Angeles County Fire Department confirmed to ET that a small plane went down and was "engulfed in flames."

"There was a private company that was working on the street and they witnessed the aircraft go down. They responded with fire extinguishers. They assisted the victim from the wreckage," the LAFD said. "By the time our units arrived, the one victim was transported to a local trauma with life-saving procedures being performed."
US Weekly identified the victim as the former reality show contestant.
CBS Los Angeles reports the private company was Best Drilling and Pump, Inc. and interviewed one of the employees, Byron Mayes, who said he witnessed the ultralight Airborne XT-912 aircraft crash just outside of Hawthorne Municipal Airport.
"As soon as the plane hit the curb, it blew up. And the flames just kept getting bigger and bigger," Mayes told local CBS. "We pulled her out of the plane before we even got to the fire extinguisher. Her legs were on fire. That’s when my partner ran in and pulled her out."

"We checked her pulse, and sadly to say there was no pulse at first," Mayes added. "But the paramedics were doing as best as they could to keep her breathing."
He also noted the pilot was wearing a Pacific Blue Air jumpsuit, and Storm posted on Facebook that she completed her first solo flight with the company in November. The Hawthorne Police Department later confirmed in a statement that the pilot did not survive the crash.
